We are seeking a full-time motivated and detail-oriented Case Manager to serve as coordinator for all internal case management activities for its Healthy Marriage and Relationship Education (HMRE) program in Darlington County inclusive of intake and enrollment eligibility to help shape service delivery strategy for program participants.
Candidate must possess a Bachelor’s degree and two years program experience in health, education, or social services and superior communication skills, strong experience working with young families or an equivalent combination of education and experience. Candidate must have a thorough knowledge of the community health & service delivery system in Darlington County, demonstrated leadership and strong work ethic and commitment, exceptional communication skills, high level of responsibility and accountability, as well as, personal warmth skills. Candidate must also possess a valid driver’s license and/or have access to a vehicle in good working condition. Candidate with bilingual skills set; Darlington County residence is preferred.
