SUMMARY OF POSITION: The Case Manager position provides high-risk intensive case management duties to ensure long-term support of housing, employment, counseling, and mentoring objectives and networks are established and carried out for Young Adult participants. The Case Manager conducts home visits periodically. Assess, formulate, and implement appropriate immediate and long range treatment plans for clients based on initial and continuous assessment of Individual Service Plan (ISP); This position will work closely with all ACER and Young Adult Staff, Program Consultants, and Community Partners to ensure that participants receive all services identified through the ISP process. The case manager requires formal education training in case management and client services. Minimum Qualifications (Two Years Prior or related Experience Minimum) • Bachelor’s Degree in Social Work or Criminal Justice, or a related field (or equivalent experience); • Certified Community Health Worker a plus; • Excellent verbal, written and interpersonal communication skills; • Computer literacy, including knowledge of basic software applications and familiarity with the internet and email communications; • Must adhere to random drug tests • Must have reliable transportation and drivers insurance and available to travel throughout the service area • Must be able to pass background check to provide services inside Prison and Work Release Facilities
OTHER SKILLS AND REQUIREMENTS: • Must be flexible to work evenings and weekends, as scheduled. • Requires excellent communication skills including written, verbal, interpersonal, decision-making, negotiation, mediation, active listening, and presentations, organized, proficient in Microsoft Word, Excel, and Power Point. • Work well with others and demonstrate team work.
